PURPOSE OF THE POST

To enhance WCO’s ability to provide technical support to the Government in scaling up the capacity of the Ministry of Health and Sanitation, at all levels, to address the health systems priorities as pertains to health financing and human resources for health in Sierra Leone.

Background

The Ministry of Health and Sanitation has developed Universal Health Coverage (UHC) Roadmap which they are desirous to implement to lead to UHC for the population of Sierra Leone. The Government of Sierra Leone is keen in implementation innovative health financing mechanisms including Sierra Leone Social Health Insurance (SLeSHI) scheme with the vision of moving towards universal health coverage. SLeSHI Act was passed and launched in 2017 with plans to start implementation immediately but yet to be operational. A secretariat has been established under the Ministry of Health and Sanitation to operationalize and roll out of SLeSHI. The vision of SLeSHI is comprehensive including all types of public and private financing. It is supposed to lead to revenue increases and improved expenditure management including efficiency by expanding access of quality service delivery in a sustainable manner.





Health care financing is central to health sector governance of Sierra Leone. It involves a process by which funds would be mobilized from primary (households and firms) and secondary sources (Government, donors and corporate bodies). These funds are to be accumulated in fund pools and are used to purchase services and products that promote, maintain and restore health.

Achieving the health system goals of improving health outcomes, providing financial risk protection and increasing system responsiveness requires direct contributions from health financing and clear relationships to other health systems functions.

Sierra Leone has very low health worker: population density of 12.3 per 10,000 population, compared to a global recommended threshold of 45 per 10,000 identified as generally necessary for the attainment of a high level of service coverage compatible with attainment of universal health coverage. Strategies and plans are being implemented by government to increase production, distribution and retention of human resources for health. As part of the process to improve HRH in Sierra Leone, postgraduate colleges of health specialists was inaugurated and started operation with admission of fellows.

The other greatest challenge in Sierra Leone relates to absence of a comprehensive and accurate data base on the health workforce, particularly pertaining to numbers, distribution, historical trends, and attrition.

All these strategic approaches are within Sierra Leone UHC Road Map which have to be implemented and monitored towards attaining the set UHC goals





DESCRIPTION OF DUTIES

The responsibility of the national SSA will be to: Health financing roles:

Technical support for development, implementation and M&E of the National Health Financing Strategy in the context of global/regional Health Financing priorities and/or strategies

  • Formulation and implementation of policies and strategies on good governance for transparency, accountability and inclusiveness
  • Pursuing equity in health financing with special emphases on; risk pooling by targeting resources to services for the poor, vulnerable groups, and public health interventions to reducing catastrophic costs of care
  • Implementation of practices for high quality health care service delivery
  • Innovation in resource mobilization
  • Advocacy for the promotion of financial risk protection and equity with special focus on the poor and vulnerable
  • Promotion of performance- based service delivery which translates into high value for money.
  • Technical assistance for strengthening national HRH information system including national health workforce observatory and national health workforce accounts in the context of health information system.
  • Technical assistance in other health systems strengthening related activities.





Human resources for health roles;

Technical support for development, implementation and M&E of the National HRH Policy and Strategy in the context of global/regional Human Resource for Health priorities and/or strategies

  • Technical assistance in the development and implementation of effective HRH management practices for recruitment, deployment, retention, including management of migration, and health security
  • Fostering national education system for production of all types of health workers, with appropriate skills and competencies in order to deliver integrated package of essential health services;
  • Technical assistance for strengthening national HRH information system including national health workforce observatory and national health workforce accounts in the context of health information system

Technical supervision

Supervised by the Coordinator, Health Systems, under the overall guidance of the WHO Representative for Sierra Leone, and working closely with counterparts at the Regional Office and HQ, the incumbent will provide technical support to the Government in scaling up the capacity of the Ministry of Health and Sanitation, at all levels, to address the above health systems challenges as pertains to health financing and human resources for health in Sierra Leone.

REQUIRED QUALIFICATIONS

Education:

Essential : First university degree in Social Sciences, Health economics, Health Financing, Health Systems or public health from an accredited/recognized institute





Desirable :

  • An advanced university degree in health systems related field, Human Resources Management, Health Economics or Public Health

Position Overview

PIH is seeking an experienced physician to help manage clinical programs at PIH-supported healthcare facilities in Sierra Leone. The Pediatrician will be based out of the Koidu Government Hospital (KGH) in Kono District. He or she will work closely with the Sierra Leone Ministry of Health and Sanitation on the development and implementation of work plans, medical education initiatives, and improving the quality of care. This individual will provide support to the Medical Director and other PIH leadership and advance PIH’s core mission of health systems strengthening in Sierra Leone.

Essential Duties and Responsibilities:

Clinical Supervision and Support

Serve as Koidu Government Hospital (KGH) Pediatric lead

Complete daily rounds on pediatric and nutrition wards at KGH with CHO and nursing staff

Manage pediatric cases according to national and international clinical guidelines

Oversee the inpatient therapeutic feeding center and outpatient therapeutic feeding program

Perform clinical teaching, mentorship and capacity building of clinicians through participation in district hospitals, outpatient clinics, community health clinics, case review, and other activities.

Provide general support and supervision throughout the hospital, as needed

Pediatric wards, Nutrition Wards for clinical supervision

Direct supervision for CHO rounding on pediatric ward

Provide supervision to social workers, in conjunction with nursing department.

Work in collaboration monthly and as needed with Medical Director/Nursing Director and Director of Operations on guiding KGH strategy.

As the KGH Pediatric Lead, serve as the point of contact for the following:

Pharmacy and procurement: use Open Boxes on an as-needed basis to support interface between clinical and pharmacy teams for pediatric formulary. Attend weekly pharmacy meetings.

Visitors and other meetings/events at KGH, as needed from senior management

Referral team – supporting Director of Nursing and Social Work as needed for pediatric patients requiring referral for diagnostic or specialty services





Administration

Attend weekly Clinical Ops Meetings at KGH

Attend KGH morning report

Approve overtime sheets and vacation request for CHOs and SWs

Work closely with the PIH SL Medical Director, Nursing Director and Kono Leadership team on strategic planning and budgeting for our pediatrics and malnutrition work.

Work with the leadership team to contribute to grant proposals and reports to current and potential donors

Education

Assist with and collaborate on supervision, curriculum building and implementation/scheduling of nursing and CHO

Continue ongoing education curriculum for CHOs

Provide bedside training for all cadres of staff

Provide clinical accompaniment for KGH/MOHS medical officers

Hold a medical degree with an up to date practicing license with the Medical and Dental Council.

Should not be receiving salary from the national consolidated funds.

PURPOSE OF THE POST

The National SSA will provide coordination and technical support to the Directorate of Nursing & Midwifery and the National Task Force for Strengthening Midwifery Education to effectively plan, organize, coordinate and engage key stakeholders in the operationalization of the Framework for Action: Strengthening Quality Midwifery Education for UHC 2030 in Sierra Leone. This includes all processes leading up to a new harmonized revised national midwifery education curriculum, in close coordination and consultation with international resource persons and the WHO Country Office.

Background

The World Health Organization Sierra Leone Country Office (WCO) is responsible for providing technical advice to the Ministry of Health and Sanitation (MOHS) in guiding overall efforts to improve the health and well-being of the population. Updating of the curriculum for midwifery education is part of the Government of Sierra Leone’s commitment to invest in high-quality education and training for midwives thereby improving the quality of sexual, reproductive, maternal, neonatal and adolescent health.

The aim for this curriculum review & development is to build a nationally uniform midwifery education which in the long term a prerequisite for a united midwifery workforce. All midwifery students will receive the same education in line with ICM Essential Competencies for Midwifery Practice, so that the needs of women and newborns are met. The curriculum will be reviewed based on the midwife’s role, responsibilities and function, which is described under the international definition of midwife.




DESCRIPTION OF DUTIES

The responsibility of the national SSA will be to:

i. Support the Directorate of Nursing & Midwifery to be an effective Secretariat for the operationalization of the Framework for Action: Strengthening Quality Midwifery Education for UHC 2030 in Sierra Leone

ii. Work with the national task force and Directorates of Nursing & Midwifery and of Training and Research to develop a workplan

iii. Ensure that all meetings are minuted, and follow up actions are implemented by responsible parties

iv. Conduct a mapping of Midwifery Education Stakeholders to ensure these are systematically involved in all processes

v. Collate all relevant documents including but not limited to; existing Sierra Leonean and regional (WAHO) Midwifery curricula, CPD framework, service scheme, scope of practice, and all the most recent national guidelines related to midwifery care and sexual and reproductive health rights, quality improvement; ensure these are available and appropriately referenced in the curriculum development process

vi. Support the development of tools for all related assessments

vii. Participate in all data collection exercises

viii. Provide technical leadership to increase awareness of the cultural context, specificities and implications

ix. Participate in relevant technical working groups in areas of quality management / improvement and information sharing

x. Support the development and review of all products and deliverables, including documentation of the process

xi. Regularly monitor and provide progress reports against the workplan

xii. Work alongside and mentor counterparts in programme management

The ultimate responsibility for ensuring key stakeholders are engaged and contribute to the development of a plan and operationalization of the Framework for Action: Strengthening Quality Midwifery Education for UHC 2030 in Sierra Leone lies with the MoHS of the Government of Sierra Leone

The Moving Integrated, Quality Maternal, Newborn, Child Health and Family Planning and Reproductive Health (MNCH/FP/RH) Services to Scale (MOMENTUM) 2A (M2A) is a five-year global project funded by the U.S. Agency for International Development (USAID) to provide targeted MNCH/FP/RH technical and capacity development assistance (TCDA) to countries to facilitate countries’ journeys to self-reliance. M2A also aims to contribute to global technical leadership and learning, and USAID’s policy dialogue for achievement of global MNCH/FP/RH goals through support to globally endorsed MNCH/FP/RH initiatives, strategies, frameworks, guidelines, and action plans.




In Sierra Leone, the project will continue on with long-term follow-up activities to sustain and improve integrated health service delivery. This position will support the initial response efforts related to infection prevention and WASH services in prioritized healthcare facilities. The position will also support the longer-term recovery and sustainability efforts to improve the quality of healthcare services, with a focus on maternal, newborn and child health services and infection prevention.

Momentum Country Global Leadership (MCGL) project is a five-year global project funded by the USAID to provide targeted MNCH/FP/RH technical and capacity development assistance (TCDA) to countries to facilitate countries’ journeys to self-reliance. MCGL also aims to contribute to global technical leadership and learning, and USAID’s policy dialogue for achievement of global MNCH/FP/RH goals through support to globally endorsed MNCH/FP/RH initiatives, strategies, frameworks, guidelines, and action plans. In Sierra Leone, the project will begin as a six-month COVID-19 rapid response effort led by Save the Children.





This position will support the government efforts in healthcare system strengthening especially with a focus on RMNCH services at the district level. S/he will represent the project at district level forums, meetings and consultations on policy and technical issues related to the project’s objectives. S/he will work closely with both the technical and programmatic teams based in the field office and country office. S/he will ensure the regular monitoring of indicators and coordinate with relevant stakeholders at the district and national level.

The PHO will be reporting directly to the District Health Coordinator. She/he will play a key role at district level in ensuring that MCGL is well represented and that MCGL is always meeting key project deliverables in a timely manner. She/he will be based in either Pujehun /Kailahun/W-Urban/ W-Rural district and will be operating from the SCI offices.

With the dual programming approach that SCI has, in the event of a major humanitarian emergency, the PHO will be expected to work outside the normal role profile in order to support the emergency response and will be expected to ably vary working hours accordingly.

Position Overview

The Midwife is a critical member of the PIH Sierra Leone team and will work collaboratively with the clinical and support services staff to provide high-quality, comprehensive maternal and newborn health services at KGH and in Wellbody (WBC) Maternal Health clinic as required. The Midwife when based at WBA, will be supervised by the Midwife Manager and at KGH by the Midwife Mentors, MOHS Sister in charge of maternity department with oversight by the Reproductive Maternal and Neonatal Health program manager. She will work to improve the quality of maternal and newborn health services in Kono District and reduce maternal and neonatal morbidity and mortality with a specific focus on labor and delivery services at KGH and WBC. Because PIH Sierra Leone is a growing organization and the Maternity Unit is a busy facility built on teamwork, the Midwife will also be expected to assist in other clinical areas as needed under the direction of the PIH RMNH program manager and Director of Nursing.





Essential Duties and Responsibilities:

Provide high quality antenatal, intrapartum and postnatal care to pregnant women, new mothers and their babies, including provision of the full package of antenatal care services as per the Sierra Leone national protocols and guidelines for emergency and obstetric and newborn care.

Actively participate in the identification and management of antenatal complications requiring treatment, follow-up and clinical management.

Provide high quality and timely basic emergency care for obstetric complications during antenatal, intrapartum and postnatal periods.

Provide high quality normal and delivery care including ensuring proper monitoring of labor through the use of the partograph and other monitoring tools in use in the department.

Supervise clinical maternity staff at the department, ensuring all patients are properly cared for and monitored, vital signs checked, medications given as prescribed, partograph’ s completed accurately and acted on as needed.

Provide daily essential newborn care to all infants born at the Maternity department at KGH and/or Wellbody Delivery Center. Identify, liaise with the neonatal team and transfer sick babies as needed to Pediatric resus for further treatment as needed.

Provide routine daily postnatal care for women and infants through to 6 weeks postpartum.

Evaluate postnatal patients prior to discharge and provide discharge counseling to ensure that mothers are able to care for their newborns competently and confidently.

Ensure proper documentation is maintained in all maternal and newborn records, health registers and clinic registers.

Collaborate with the RMNH program manager and the rest of the staff to ensure clinical staff can provide CEmONC services in line with MOHS and WHO standards.

Prepare documentation and participate in MDSR monthly reviews, including presentations for KGH and DHMT.

Provide clinical assessment, birth plan development, and on-going care for patients at the Wellbody Birth Waiting Home if required.

Provide leadership, supervision and support for maternity staff to provide quality maternal and newborn health services at KGH and Wellbody Clinic including BEmONC/CEmONC services, ANC, PNC, Delivery Care, Essential Newborn Care, PMTCT, AYFS and Family Planning.

Lead and enforce health & safety standards, Infection Prevention and Control protocols, including ensuring the availability of PPE and cleaning supplies.

Ensure and maintain daily stock checks for the availability of medicines, consumables, supplies and equipment for the Delivery Department to avoid stock outs, including during nights and weekends.

Support maternal and newborn health outreach activities in collaboration with the RMNH program manager.

Collaborate with the RMNH program manager, Obstetrician and the Midwife mentors to provide training on maternity care for clinic staff.

Work rotating shifts, including significant night shift coverage, and rotating “on call” weekends.

Provide compassionate and respectful maternity care in line with the PIH mission and values.

Participate in all quality improvement activities in the department and significantly contribute to successful outcomes and targets being met.

1.) State Enrolled Community Health Nurse (SECHN)

Position Overview

The State Enrolled Community Health Nurse (SECHN) is a critical member of the clinical staff at PIH. SECHNs work collaboratively alongside physicians, SCMs, CHOs, MCH Aides, TBAs and support staff to provide high quality care to patients and their families.

Essential Duties and Responsibilities:

Respond to medical/health problems presented by patients in the pediatric departments (including primary health) or any of the medical departments in collaboration with MoHS and DHMT staff if needed, or at any of the PIH-affiliated facilities.

Position Overview

A Community Health Officer (CHO) generally applies his/her medical knowledge and skills to the diagnosis, prevention and management of patients that flows into the Wellbody Clinic or Koidu Government Hospital and any of the Wellbody Alliance/Partners in Health affiliated health facility. He/she does this thoroughly following the principles of inspection, palpation, percussion and auscultation where ever applicable to find out the actual cause of disease, and also utilizing the appropriate laboratory work. He/she should consult the medical officers for discussion on patient’s care plan in some critical cases, respecting patient’s privacy and confidentiality as much as possible.

Covering the NCD clinic and the TB/HIV clinic if it is required.

Supply patients with information and education on their prescriptions

The Aberdeen Women’s Centre (AWC) is a Non-Governmental charitable hospital located in Aberdeen, Freetown providing free surgical services to the women of Sierra Leone for correction of disorders arising from protracted labor; we are also providing free maternity services and an outpatient pediatric clinic for children between birth and fifteen years old.




For its operations, AWC is looking for qualified Sierra Leoneans to fill the following position:

Position: Part Time Obstetric Cover Doctor (Evenings and Weekend Cover)

Purpose of the Role

The part time Obstetric cover doctor will be responsible for the clinical care in the hospital during their time on call. The work up of patients from history taking, examination, ordering and interpreting investigations, along with assessment of fetal well-being is expected in this role. As a maternity unit, we provide not only maternal care, but neonatal care also. This role requires the ability to undertake caesarean sections, instrumental delivery, perineal repair and manage the obstetric emergencies. It requires an insight as to when help is necessary, and ability to reach out to those around you for escalation of care.




 

This role also requires leadership of the clinical team while on duty, and liaison and effective communication with the senior cover is expected. The Obstetric doctor takes responsibility for all patients and their babies that come to the hospital during their time on call.

Responsibilities and Duties

Clinical

  • Shift hours start promptly at 5:00 pm Monday to Friday and 8:00 am on Saturday and Sunday. The shift is finished when you have physically handed over the patients to the next duty doctor. Ideally handover should commence at 0730 to allow the night doctor to leave by 8am.

  • Oversee daily activities in the maternity

  • It is sometimes necessary to assist with fistula patient care, but this should be in close liaison with the fistula surgeon.

  • Provide consultations and treatment to AWC patients including referral when necessary.

  • Responsible for prescriptions of drugs for patients.

  • Ensure clear and effective handover of all antenatal patients, labouring mothers and “problem” postnatal patients is done at the beginning and end of the shift

  • Ensure that ward rounds are done during the shift hours. Also, to ensure that a walk through of the labour ward occurs at least every 4 hours during your shift.

  • Decisions regarding a patient should be undertaken after sufficient information has been obtained, and if instruction over the phone is given, follow up clinical assessment of high-risk patients in person is always necessary. (Please also see policy to for definitions of “high risk”)

  • Decisions of antenatal patients should be taken in conjunction with the senior AWC doctor cover. Do not discharge high risk patients without discussing the case first with the senior AWC doctor.

  • Provide consultations and treatment to AWC staff during shift hours.

  • Provide emergency neonatal assessment, care and support, and arrange for transfer to SCBU if necessary. If transfer is indicated to complete a record in the neonatal transfer file, so we can follow up.

  • To tend to the emergency and medical needs of an SGBV patient (for example, Malaria, or head injury), and alert the senior AWC doctor of the patient’s arrival so s/he can complete the pelvic examination and documentation.




Supervision of staff

  • To provide clinical support as well as team leadership to staff, encouraging healthy team dynamics, morale and good overall relationships.

  • Ensure that staff are punctual during shift hours and attend for their respective handovers.

  • Ensure staff are fulfilling their duties and escalate if necessary; examples include frequent fetal monitoring, vital signs monitoring and completing the pantograph.

  • Address staff issues that arise during shift hours, i.e. interpersonal conflicts or clinical performance issues that are witnessed during the shift. This should be done in conjunction with AWC senior doctor and relevant supervisors.

 

Administrative

  • Ensure that patient records and charts are completed and properly filed.

  • Liaise with OPC, Lab, Pharmacy, Stores departments regarding safety and health of patients and staff. Escalate concerns to senior AWC doctor cover.

  • Ensure that supplies required for patient care and staff usage are available.

  • Update and inform management / consultant Obstetrician on any incidences that arise during the work shift.

Private consultations to non AWC registered patients are not allowed at AWC premises during shift hours.

Reporting to the Chief, Health and Nutrition and working with the Supply and Logistics Section, the Health Specialist (Supply Chain System Strengthening) provides professional technical, operational, and managerial support for the supply component of health and nutrition Programme within the UNICEF Sierra Leone Country Programme. The Health Specialist (Supply Chain Strengthening) pays a special attention to the management of emergency supplies, including tracking, monitoring, and reporting of the distributed items up to health facilities as feasible. At the same time, with a view to enhancing the humanitarian-development nexus, the Health Specialist (Supply Chain Strengthening) also provides targeted capacity building support and technical assistance to the Ministry of Health and Sanitation (MoHS) in strengthening overall supply chain strategy, forecasting and supply planning, inventory management, warehousing and distribution, implementation of logistics management information system and data analysis/utilization as per the identified needs. To qualify as an advocate for every child you will have…

  • An advanced university degree in public health/nutrition, pharmacy, pediatric health, family health, supply chain and logistics, health research, global/international health, health policy and/or management, epidemiology, or any other relevant technical field.
  • A minimum of eight years of professional experience in health supply chain management, public health/nutrition planning and management with supply component, or maternal and neonatal health care with supply chain management.
  • Additional professional certification (CSCP, CSCM, CPSM, SCMP, etc.) from an accredited supply management institution in conjunction is an asset.
  • Experience working in a developing country is required.
  • Experience in emergency preparedness and response is considered as an asset.
  • Professional technical knowledge/ expertise in supply chain system, including forecasting and supply planning, procurement, inventory management, warehousing and distribution, logistics management information system.
  • Expert knowledge of the technical areas of UNICEF health and nutrition programmes.
