Our Work In Sierra Leone:

Partners In Health (PIH) began supporting the government of Sierra Leone in response to the Ebola epidemic in October 2014. Recognizing the large need for continued investments in the health system after Ebola, PIH is committed to long-term health system strengthening in support of the Ministry of Health and Sanitation (MOHS), guided by the same principles as our emergency response and our work around the world: public sector accompaniment, integrated and comprehensive services, social justice, nimbleness and flexibility, partnership, community ownership, and evidence-based interventions. In the aftermath of the epidemic, PIH is committed to supporting the delivery of comprehensive health services in Sierra Leone, from the community level to the health center and to referral hospitals.

Position Overview

The State Enrolled Community Health Nurse (SECHN) is a critical member of the clinical staff at PIH. At Primary Health Care, the SECHN is expected to be versatile and be ready to take on responsibilities in various departments, and also work collaboratively alongside different members of the health care team.  The SECHN will provide high quality nursing care to patients and support to their families and other caregivers.  He/She will respond in a timely manner to patients who urgently need treatment as well as provide health education to patients and caregivers. The SECHN will also ensure accurate and proper documentation of all patient care activities, participate in Quality Improvement projects and be involved in clinical education sessions for professional development.

Essential Duties and Responsibilities: General Nursing:

  • Triage patients presenting to the clinic based on their level of acuity;
  • Ensures that patients presenting to the clinic are managed appropriately including identifying those who need transfer to a higher level of care.
  • Obtain and document patients’ vital signs, recognizing and responding to abnormal vital signs appropriately.
  • Maintain proper patient records through effective documentation.
  • Provide health education to the patients and their care-givers.
  • Deliver high standards of care in collaboration with the rest of the clinical team.
  • Responsible for the preparation of monthly reports to be submitted to the appropriate departments for further analysis. Display high level of commitment by being adaptable and being able to function in a busy work environment, including rotating to other units at the clinic as required
  • Maintain confidentiality and impartiality and involve families in the plan of care.
  • Promote patient/family-centered care to ensure that their needs are met.
  • Engage and participate in clinical education or in-service training as determined by the Clinical Lead of the facility, Nurse Educator or the Director of Nursing.
  • Participate in quality improvement projects and other related studies that could improve on the delivery of care to patients.
  • Perform other duties related to the role, as may be required by the Clinical Lead or Director of Nursing.

Nutrition and Feeding: 

  • Ensure that all patients are receiving the right diet according to age and diagnosis.
  • Encourage lactating mothers to practice exclusive breastfeeding.
  • Ensure that Severe Acute Malnutrition (SAM) patients are receiving their supplies according to the protocol.
  • Ensure that motherless infants are receiving their supplies according to the protocol.
  • Ensure that Moderate Acute Malnutrition (MAM) patients and care-givers are educated on the preparation of local diet.

HIV/TB Clinic:

  • Promote continuous medical education among other clinical staff.
  • Conduct pre- and post- diagnosis counseling for patients, as well as education on adherence to treatment.
  • Perform any other duties as assigned by the department lead or Clinical Lead of the facility.
  • Ensures that all emergency cases are prioritized and seen appropriately regardless of the time of the day.
  • Be willing to work or rotate to other department as determined by your supervisor or Clinical Lead.

NCD Clinic:

  • Provide NCD care in the NCD clinic (Diabetic Management, Management of High Blood Pressure, Management of Hepatitis B);
  • Ensure proper follow-up care for patients scheduled to return to the clinic.
  • Organization of the NCD clinic to ensure that it operates efficiently.
  • Rotate to other departments as the need arises.

Education and Work Experience Requirements: 

  • Current and valid clinical license, (SECHN Certificate and License).
  • Clinical experience in Maternal and Child Health, HIV/TB, NCD and General Nursing preferred.
  • Strong communication, interpersonal, and organizational skills, must be able to take initiative and problem-solve, and must be flexible.
  • Ability to work well with a team, be dependable, and hard-working.
  • Ability to be adaptable and function well in a busy work environment, including rotating to other units as required.
  • Commitment to social justice and human rights.
  • Demonstrated understanding and respect for PIH’s mission and values.
  • Must NOT hold dual employment from private or national consolidated funds.

Position Overview

The primary responsibility of Medical Officer for Internal Medicine and Emergency care is to carryout successful medical services in our Koidu Government Hospital (KGH), perform daily operations and provide supports, and as well receives directions from the Internal Medicine consultant.

Essential Duties and Responsibilities: 

  • Serve as Koidu Government Hospital (KGH) Internal Medicine Medical Officer within the Internal Medicine and Emergency department and under the supervision and mentorship of the PIH Internal Medicine consultant
  • Daily rounds on the Medical wards, Emergency department and NCD clinic at KGH. This position will be supported by more senior PIH Internal Medicine consultant to develop expertise and knowledge in Internal Medicine, Critical care and Infectious diseases.
  • Supervision of the CHOs in the medical ward, Emergency department and NCD clinic including provision of clinical mentorship and continuous training in collaboration with the PIH Internal Medicine consultant.
  • Management of medical cases per national and international clinical guidelines.
  • Work with the PIH HIV/TB Clinical Officer to provide supportive supervision to MOHS health workers for HIV/TB services at KGH (including case detection, counseling and testing, HIV case management, diagnosis and management of Opportunistic infections according to national guidelines).
  • Participation in district hospitals, outpatient clinics, and community health clinics case reviews, and other activities that may increase the knowledge and expertise in Clinical Medicine with special interest in Internal Medicine, Critical care, Infectious diseases and NCDs, as required.
  • The Medical Officer is also expected to actively participate in and support, PIH program systems, such as the MEQ data collection systems and feedback loops, the KPI presentations and discussions besides other general systems that will help support the General clinical outcomes and the Internal Medicine Clinical Outcomes in particular.
  • The Medical Officer at the Internal Medicine Department, in collaboration with the PIH senior consultant and the PIH Clinical Educator is also expected to actively make inputs to the clinical trainings curriculum for other cadres of clinical staff at KGH, WBA and other CHCs that PIH may be supporting.
  • Interest in Clinical and Academic personal development. The candidate will be supported to join the National Post graduate training program for Internal Medicine at MoHS after this posting.
  • Be willing to work in any other department as needed but within your capability, and required by the PIH Clinical leadership.
  • Clinical work:
    • Daily rounds at the Medical wards (Male, female), Infectious diseases Unit, Emergency department, NCD clinic at KGH.
    • Work with the clinical staff of WBA and any CHC that PIH is supporting for referrals, specialized medical advice and training/mentorship.
    • Collaboration with CHO rounds
    • Provide support to nurses and social workers, in conjunction with nursing department.
  • As the KGH Internal Medicine Medical Officer, serve as the point of contact for the following:
  • Support of the National programs for malaria, TB, HIV, NCDs, Hepatitis B, Emergency and critical care and Infectious diseases (COVID 19, Ebola, Lassa Fever, etc). under the supervision of the PIH Internal Medicine consultant
  • Referral team – collaborate with the  Nursing & Social Work Program Manager as needed for patients requiring referral for diagnostic or specialty services
  • Administration
    • Work closely with the PIH Internal Medicine consultant, Chief Medical Officer, clinical team and Kono Leadership team on strategic planning and budgeting for our Medical programs.
    • Attend daily morning briefing/meeting at KGH as required

 

In addition, specific interest in continuing the collaboration with MoHS and the Post Graduate training program in the country will be highly valued.

This job may give an opportunity of developing work in the field and expertise for the next movement forwards in the Clinical training.

Education and Work Experience Requirements 

  • Medical Officer Certification per the Sierra Leone training program.
  • Experience in HIV/TB, Malaria and other infectious diseases, Emergency Medicine and Non-communicable diseases preferred.

  • Ability to be flexible and effective in the face of unexpected challenges.
  • Exemplary interpersonal communication skills, verbal and written, required to effectively engage with senior leadership, partners, and clinicians on sensitive political and confidential issues.
  • Proven ability to exercise good judgment, diplomacy, demonstrate political astuteness, and tact.
  • Sense of humor and flexible disposition very helpful.
  • Willingness to live in Kono district full-time.
  • Computer basic skills: Office, Internet and data base systems.

 

Additional skills/ knowledge/experience preferred:

  • HIV/TB clinical and program management strongly preferred
  • Experience in treating malaria and other prevalent Infectious diseases (COVID19, Ebola, Lassa, Hepatitis B)
  • Experience in Emergency and critical care
  • Experience in NCDs (i.e. Hypertension, diabetes)
